FINAL PROJECT 2:Directions: The final project for OSH 366 will be a video presentation and PowerPoint (PPT) of you completing a Job Hazard Analysis (JHA). You will be making recommendations and providing a training program. Videos are preferred, but narrated PowerPoint presentations will satisfy the minimum requirements. Students should use the notes section of PPT to incorporate their comments as to what they would say in their training to their audience. FAILING TO ADD NOTES IN THE NOTES SECTION CAN BE A DEDUCTION OF AT LEAST 30% OFF THE TOTAL SCORE.Using your JHA forms, identify a job which has a hazardous task to complete. Please supply a brief description of the job and of the task they will be performing. *You are then going to either 1) take a short video of the task OR 2) take photos of the task being performed. You will then add the video/photos to a PowerPoint presentation.The video, or photos of the “job” you select should be a job that is in your organization that presents challenges to be completed safely. The job should have ONE task listed, the most critical or hazardous, which has 1) at least 10 steps to complete it AND 2) hazards that require controls, such as PPE and administrative controls (based on the hazards associated with it, such as chemical, biological, electrical, mechanical, etc.). If you do not have a job like this in your facility, then try to find one. NOTE: IF YOU DO NOT HAVE AUTHORIZATION BY YOUR EMPLOYER TO RECORD VIDEO OF A JOB–TASK YOU WILL HAVE TO USE A YOUTUBE VIDEO AS A REPRESENTATIVE EXAMPLE. The task can also be associated with a confined space entry, Lockout/Tag out procedures, life-safety, asbestos exposure or Bloodborne pathogens, the control of chemical hazards or noise hazards, as set forth in OSHA’s Hearing Conservation Program.
SECOND, use the JHA form to describe the task being completed in 10 or more steps. Make sure to identify the most critical/hazardous steps of the job. After this has been done, identify the hazards of the task steps and the controls used to reduce or eliminate them. If there are any problems you identify during the JHA note them and how you will fix the problems found.This assignment will be completed in two phases and will consist of 300 points total. (JHA Narrative and Forms = 150 points; Final Project Training PPT = 150points).

You can either record your voice onto each slides OR you can write the words you are stating for each slide under each slide in the notes section. Be specific… Write as much information as possible. Do not write one sentence under each slide… Remember you are training me. Anticipate questions I may ask and address them. Convince me of the hazards and how to prevent injury or illness. Use the guide I provided to the flow of the PPT.
