The incorporation of multiple perspectives in health care offers the benefit of diverse knowledge and experience; however, in practice, shared responsibility without high-quality teamwork can be fraught with peril. For example, “handoffs,” in which one clinician gives over to another the primary responsibility for care of a hospitalized patient, are associated with both avoidable adverse events and “near misses,” due in part to inadequacy of communication among clinicians.8-12 In addition to the immediate risks for patients, lack of purposeful team care can also lead to unnecessary waste and cost (Mitchell et al., 2012).

The perils that the authors say lead to uncoordinated care are:

1.Inadequacy of communication among clinicians. effective teams are characterized by common purpose and intent, trust, respect, and collaboration. Team members value familiarity over formality and watch out for each other to make sure mistakes are not made. Health care teams that do not trust, respect, and collaborate with one another are more likely to make a mistake that could negatively impact the safety of patients (Hughes, 2008).

2.Lack of purposeful team care. An effective teamwork is now globally recognized as an essential tool for constructing a more effective and patient-centered health care delivery system. Nowadays, patients are rarely looked after by just one health professional. In the context of a complex healthcare system, an effective teamwork is essential for patient safety as it minimizes adverse events caused by miscommunication with others caring for the patient, and misunderstandings of roles and responsibilities ( Babiker, 2014).

How communication and interdisciplinary collaboration prevent adverse events:

Fundamentals of effective team communication include the active membership of the patient and family and the willingness and capability of team members to be clear and direct and communicate without technical jargon. Information sharing is the goal of communication, and all team members need to recognize that this includes both technical and affective information (Mitchell, 2012). Effective extraprofessional communication is communication that occurs in an open, collaborative and responsible manner. It is communication that is built on mutual trust among healthcare professionals/providers/learners, patients and their families
