Post an assessment of Magda’s situation that addresses the seven domains of biopsychosocial assessment of older adults.
Magda is an 81-year-old widowed female who at once was self-sufficient. Magda fell and broke her hip and was then treated for this fall. After that, Magda was diagnosed with early signs of dementia. Her son, his wife, and their son Alec began taking care of her after the wife decided that it would be best to hire in the family than to pay someone else. After assessing Magda with the biopsychosocial, she lacks physical well-being and health, cognitive capacity, ability to perform ADL’s, and safety in her home.
Magda’s physical well-being came to a downfall after breaking her hip and having to have surgery. After the incident with breaking her hip, she was diagnosed with early signs of dementia, which is where she lacks cognitive capacity. At one point in time, she was able to perform activities of daily living, but now she cannot. She needs the help of a caregiver, in which they once had, but they decided to hire her grandson, Alec. Now, Alec serves as a safety hazard to her in her home. According to Elman et al. (2020), elder mistreatment is perpetrated by a person in a position of trust or by someone who targets an older adult based on age and disability.
Describe ways you would have supported Magda’s self-determination through the assessment and treatment-planning process. Include questions you would have asked Magda and her professional support system to gain further insight into the situation.
As a social worker, I would have allowed Magda to make her own decision about who she wanted her caretaker to be. For the sake of her well-being and her personal wants, this would have been first in the assessment due to the fact that she still wanted to be able to attend church and other church activities. This reflects on the dignity and worth of a person, according to NASW Code of Ethics. Prior to this, I would have gathered appropriate information from the health care givers before leaving the facility. Gathering any kind of health record information would have made the transition to home easier, whether it was on paper or verbal doctor’s order.

The biopsychosocial assessment of older adults is physical well-being and health, psychological well-being and health, assessment of family caregivers, cognitive capacity, physical environment, ability to perform ADLs, and social functioning (Christ, G., & Diwan, S. (n.d.).
Magda has a broken hip due to a fall. This fall has caused some concern regarding her physical well-being. Due to Magda’s broken hip, her ADL performance has decreased. The scenario stated that medication was missing but did not go into detail regarding if the medication was for pain, dementia, or other ailments. However, due to the previous fall and being under medication, she has an increased risk of falling again, and preventative measures should be addressed regarding her safety. A recommendation for occupational or physical therapy may be needed to address what task she can complete independently or with assistance.
Her grandson needs to know how often she may need to be turned or repositioned if she cannot move on her own, so she does not get bed sores. He needs to be aware of when she should be eating due to some meds that may need to be taken before or with a meal; being out all night presents a problem for Magda’s physical well-being.
She was diagnosed with dementia, but there was no mention of her cognitive capacity in detail. Are there activities that Magda could benefit from to improve her cognitive ability? What specialized training will her caregivers need to assist her effectively? None of these questions were asked, and Magda was not asked if she liked her current caregiver and how she felt about possibly having her grandson become a live-in caregiver. What qualifications does the grandson have to be Magda’s caregiver?
Magda’s physical environment should be assessed to ensure the living space is appropriate for her current health conditions. She may need specific items to make her ADLs easier. The social worker and occupational therapist could provide an assessment and recommendation of the living environment.
Magda’s son, daughter-in-law, and grandson should have also been assessed to determine if they are a sound support system for Magda and if they can give her the care she needs. I would have conducted a biopsychosocial assessment of Magda before Alec moved in and talked with Magda about her thoughts and input regarding her care plan. Alec’s background should have been addressed, and his mother should not have allowed him to be a caregiver to an older adult who was vulnerable. I would have also spoken with the discharge team regarding the care Magda would need for the following discharge.

I will apply acceptance and commitment therapy (ACT) to Helen’s case. ACT is a branch of CBT that allows the client to accept their deeper feelings as appropriate responses to certain situations. When the client has accepted their difficult situation then they may begin to move forward by committing to making the changes in their behavior or thinking, cognitive diffusion helps the client to change the way they react to upsetting thoughts or feelings and gives you the ability to reframe your automatic thoughts in response to certain situations (Blackledge, Ciarrochi & Dean, 2009).
- Briefly paraphrase, in 2–3 sentences, the methodological context (i.e., research method, how data was collected, and the instruments used) of the study and the findings.
This study involved work with 30 volunteers in a psychiatric center who were separated into groups of 15. These groups were evaluated by using research tools such as the Beck Anxiety and Depression Inventory. The data was analyzed by using the analysis of covariance. The analysis of the research data showed the difference in results between the experimental group and the control group. The result of this study saw that Acceptance and Commitment therapy does decrease anxiety and depression symptoms (Heydari, et. al, 2018).
- Explain how the findings are applicable or appropriate for the client in your case study.
Helen Petrakis is a 51-year-old wife and mother from a traditional Greek family. Her feelings of inadequacy have led her to have symptoms of depression and anxiety. These feelings have been brought on by her inability to complete the task of effectively caring for her family. Her family have made comments that she has been lacking in her performance as the matriarch of their family. Helen is currently care-giving to her husband’s mother which has left her with less time to devote to her husband and children. The study I reviewed directly applied to Helen’s situation because the symptoms being researched using adults are the symptoms that Helen is reporting.
- Determine whether you would use or not use the therapy you selected for the client in your selected case study (consider how culturally relevant it is, how aligned it is with social work ethics, etc.) and explain why.
By applying ACT Helen can accept her feelings a as natural reaction to having less time to devote to her family with the understanding that her situation may have to be this way until she’s able to find extra help with her mother- in-law or is able to convince her family to pitch in more at home to help her while she cares for her mother-in-law. Much of Helen’s feeling of inadequacy is related to her Greek culture and their family values which is something to explore with Helen through ACT to help her understand why she feels so much pressure to perform at such a high level for her family (Alexander-Stamatios & Dalla, 2015). The therapist would help Helen to commit to making changes in her thought process by helping understand where these thoughts and feelings are originating from and work toward making changes to help resolve feelings of inadequacy which is contributing to her depression and anxiety symptoms (Blackledge, Ciarrochi & Dean, 2009). For social workers working with a broad range of behavioral problems that require short, empirically based interventions ACT would be an appropriate approach. ACT has been to be effective in many different areas that a social work practitioner may encounter such as childhood trauma and abuse, marriage and family therapy, substance abuse issues, phobias and obsessive behavior (Dewane, 2012).

I have chosen the Acceptance and Commitment Theory (ACT) to use for the case study of Ella. Acceptance and Commitment Theory based program examined within the scope and to be effective on the dependent variable. The results obtained suggest using acceptance and commitment theory in protecting and preventative mental health services to prepare adolescents and young adults. Acceptance and Commitment therapy is using mindfulness program to use with adolescents compared to other mediation-based programs because adolescents may struggle to engage with mediation particularly in a school setting. It is also appealing to use with adolescents as it places particular emphasis on using mindfulness to regulate emotions, which is important for this age group. The experimental studies based on act were symptomatically reviewed the applied programs were observed to be effective on variables such as reducing social appearance, anxiety, increase making life decisions, increasing the meaning of life, gaining the ability to cope, reducing the fear of social skills, increasing body image, and reducing test anxiety (Dursun, A., & Akkaya, M. 2022).
Act can help teenagers to reframe from negativity, develop coping skills, and be in the moment in their life.
A social worker could use Act on the case of Ella. Ella will need to learn to cope and navigate through the emotions of her mom and dad. Ella is unsure if her mom is still with her dad or not. Ella may feel betrayed by her mom by allowing the abuse to go on. Ella is also concerned with her parents knowing that she is bisexual due to their religious beliefs.
As a social worker it would be essential to work with Ella on dealing with her dads abuse and her mom being in denial.
