(TOPIC TO BE CHOSEN)
Medication errors are ongoing problems among hospitalized patients and may occur during prescribing, transcribing, prescription auditing, preparing, dispensing, administration, and monitoring. Near misses are errors that happened but were captured before reaching the patient, whereas medication administration errors (MAEs) are those errors that actually reach patients and pose a threat to patient safety.i wouldbe more than happy to choose this topic for my executive summary assignment.medication errors can either cause temporary or permanant harm to the patient leading to prolong hospital stay and  even  contribute or results in  patient death.implimenting many quality improvement programs like highlighting  look alike and sound alike meds, indicating high alert meds, reducing the kind and quantity of meds stored outside the pharmacy, use of pixis machine for scanning the meds, shortning the time from stat entry order to the actual administration of meds, focussing on patient allergy list to prevent cross allergy, promoting the awareness of NG tube administration orders.these are the valuable points which i prefer to put onthe  table for  my project approval
