Practice Concern in My Area of Practice

A practice concern in my workplace would be healthcare errors due to nurse and nursing assistant shortages. At my work, there are usually 2 nurses and 4 aides per floor for around 30 patients on each floor. For about a month now my work has been lucky to have 3 aides and 1 nurse per floor with a nursing supervisor that rotates between floors as needed. My facility has held a 5-star rating through the state and other surveyors in the past and I believe this number is at risk of decreasing due to this shortage in staffing. There have been more patient falls in the last 4 weeks than we have had in the past 7-8 months prior to having a staff shortage. Personally, I feel cares for patients are rushed which places patients at risk for many things such as falls.

Evidence Based Practices

Evidence based practice (EBP) research has been done regarding nursing shortage. A study supported by the Agency for Healthcare Research and Quality was performed using HIT which estimated that nursing schools need to graduate 25 percent more nurses each year. However, insufficient nursing faculty, inadequate financial resources, limited physical capacity, and insufficient clinical education sites mean that schools today cannot produce this number of nurses. (Nursing shortage, 2011). Information technology (IT) supports EBP by allowing health care practitioners to access and evaluate clinical evidence as they formulate their patient care strategies (Wells, 2006). IT and EBP both perform immense roles in healthcare.

Evidenced Based Practice

Health care is constantly changing and evolving and so is technology. Technology can be used in many ways in health care. With information technologies and data mining patient’s health care information can be used to improve quality care. “Recent advances in data mining techniques have resulted in wider applications in medical research, such as investigations into the causes of cardiovascular disease, lung cancer and cystic fibrosis” (Yi Chao, 2013).” Evidence based practice is research based practice that is the best practice for positive patient outcomes. Overall, practitioners want to prescribe the best treatment to their patients. One concern noted by many practitioners is the overuse of opioid pain medication. Opioid medications are constantly being prescribed when there is little to no confirmed effectiveness for their therapeutic use. Even though the list of side effects and adverse reactions continues to grow, opioids are the most frequently prescribed medication in the United States. (Miller & Gold, 2015) With health information technology and data mining we can see how many opioids are being written and what common diagnoses they are being written for. The Agency for Healthcare Research and Quality (ahrq.gov) uses research and evidence to create best practice standards. Using evidence based practice can make health care safer and improve quality overall. AHRQ also tracks data in order to improve performance and to evaluate the U.S. Health system.

Practice Concern

Low back pain is a common medical complaint in the emergency department. There are multiple treatment options for acute low back pain. Pharmacological treatments, non-pharmacological treatments, and injection therapies are available. The Agency for Healthcare Research and Quality (ahrq.gov) has done research review on the best treatment options for low back pain. Most trials were conducted in patients with non-radicular low back pain or mixed populations with primarily non-radicular low back pain. The findings were varied. For acute or subacute low back pain, NSAIDs, opioids and skeletal muscle relaxants were associated with small effects on pain versus placebo, and NSAIDs were associated with small effects on function. Acetaminophen and systemic corticosteroids were associated with no beneficial effects versus placebo. Overall, pharmacological therapies were associated with an increased risk of adverse events versus placebo. Non- pharmacological therapies including spinal manipulation, heat, massage, and low-level laser therapy were associated with some beneficial effects versus a sham therapy, no intervention, or usual care. Effects on pain and function were moderate for exercise, massage, and heat. Assessment and reporting of harms for non-pharmacological therapies were suboptimal but indicated no serious harms (ahrq.gov, 2016). Providers can use research such as this to educate patients on why opioids may not be the best treatment for acute low back pain.

The practice concern in my area of practice is the increasing use of mandatory overtime as a means of staffing. This staffing method is dangerous and it is caused by, among other things, the shortage of nurses and high admission levels. The effect is that it contributes to the decreasing patient care quality and the increase in reported medical errors that put lives of patients in danger (Rogers, Hwang, Scott, Aiken & Dinges, 2004)………………………………
