The documentation process in case management includes the development of measurable goals
Prior to completing this assignment, take a few moments to review examples of measurable goals:
Demarcus is 22 years old and unemployed.
Measurable goal = Johnny will complete 3 job applications within the next 10 days and within 30 days will schedule an appointment at local college’s career services center.
Marion is 42 and having trouble with insomnia due to excessive worrying.
Measurable goal = Marion will learn and implement 2-3 relaxation techniques when preparing for evening sleep, reducing frequency of insomnia to less than 3 days a week. Marion will schedule an appointment with her physician within the next 14 days.
Jonny is 34 years old and is drinks excessively.
Measurable goal = Johnny will seek a substance abuse evaluation within the next 72 hours. Johnny will have 0 incidences of excessive drinking within 90 days.
In a one-page paper address the following:
1. Write a measurable goal for the following scenario:
Gerry is 15 years old. He is skipping school and has failing grades.
TIP: Writing that, “Gerry will get better grades” falls short of being a measurable goal. How will you know that Gerry’s grades have improved? Think numerically. Can you measure school attendance and grades? Is so, how will you do this?
2. Once you have written a measurable goal, discuss the types of services you might put in place to help Gerry attain that goal.
Submit a one-page paper in APA format that addresses all the above requirements. Support your paper with credible sources as needed. In addition to your own research, the library resources provided in this course are appropriate options.
