Purpose: The learning contract acts as a guide to help the student and the field supervisor to define what the student will learn, and the specific experiences that will be part of the placement.

Provide complete and accurate information for each of the requested fields. The description of the field assignment should include the purpose of the agency and the populations served. Include a detailed description of your responsibilities as a social work intern. The internship schedule should be listed and the time and place of the hour of weekly supervision provided by your field supervisor should be noted.
