Wellness Visit, Part 1: Patient Interview
Your final project in this course is a 3-part Wellness Visit that includes a 1) a written comprehensive patient interview (Subjective), 2) a written comprehensive head-to-toe physical examination (Objective), Assessment (Differential diagnosis) and Plan of Care, and 3) a VoiceThread voice-over case presentation of the wellness visit SOAP note.
This project synthesizes the previous weeks\’ assignments and integrates the use of Healthy People 2030 objectives, applied nursing theory, and the role of graduate-level nurse assessment skills.

Final Project
Your final project in this course is a 3-part Wellness Visit of one patient (should be the same patient used throughout the final project assignment) that includes the following:
· Part 1- a written comprehensive patient interview (Subjective),
· Part 2- a written comprehensive head-to-toe physical examination (Objective), Assessment (differentials diagnosis), and Plan of Care,
· Part 3- a VoiceThread presentation of the comprehensive wellness visit SOAP note.
· Make sure all parts of this assignment refer to one patient for consistency.
Description of the Wellness Visit, Final Project:
First Part- The Patient Interview
Note: The first part of the project will be due in Week 2.
The first part- will require a written subjective data collection of a patient. You must find a volunteer to serve as a patient for all parts of this final project assignment. Please do not use personal identification names only initials in the assignment. The volunteer patient must be over the age of 18 yrs old. This assignment will focus on a wellness visit patient interview (subjective details only). Use the rubric as a guide. The contents will include:
- Chief complaint
- History of Present Illness
- Past Medical History,
- Surgical History,
- Family History
- Ob/Gyn History
- Social history
- Allergies and Reaction
- Medications
- Health Maintenance
- Immunizations
- Review of Systems
