- After reviewing the fact sheet and visiting the CMS website, compose a two- to three-page letter (this length does not include the required title and reference pages) to a geriatrician, a community-based health organization, or a hospital in your community that encourages the development of a proposal for a Community-based Care Transitions Program (CCTP) in the community.
- The letter should include the name and address of the provider, the date, an introductory paragraph, a body, a concluding paragraph, and a signature block.
- Be sure to include some of the benefits of the CCTP to the community, to Medicare beneficiaries, and to the healthcare system.

After reviewing the fact sheet and visiting the CMS website, develop a two- to three-page outline (this does not include the required title and reference pages) of your plan to convince providers in your area to submit a proposal for funding of a Community-based Care Transitions Program (CCTP) in your community. You will take this outline with you when you meet with the different providers and use it as talking points.
Your document should include the following.
- The names and addresses of three potential providers (geriatrics provider, community-based health organization, and hospital) in your community who you believe might be interested in the development of a proposal for a Community-based Care Transitions Program (CCTP). Place this information on one page.
- An outline of talking points that you will cover during your meetings with the different providers. Place this information on one to two pages.
- The outline should include the following five basic elements with two to three sentences written about each.
