For Assignment 3, you will create a fictitious patient with your chosen chronic illness, and use the LifeCare Writer to create a discharge plan for that patient. Your paper will begin with an introduction. To address the first section of the rubric, you will cut-and-paste your assessment guide (the Life Care Plan Summary and Report). To meet the second section of the rubric, you will cut-and-paste the Life Care Plan of care (the tables in LCW). The third section of the rubric will be your interpretation: What did you learn from using the LCW? What do you think about using this tool?
