When he was admitted to the nursing home, the nurse admitting Mr. Douglas documented that he had a pressure ulcer on his sacrum. She described this pressure ulcer as measuring 3 cm by 5 cm by 0 cm. The wound bed presented with 100% yellow necrotic tissue, a moderate amount of drainage, and a foul odor. He was underweight, and his albumin level was 2. He was admitted to the nursing home for physical therapy to provide a program to improve strength, balance, and mobility after his long hospital stay.
